Supporting Integrated Care Systems in End of Life Care
Integrated Care Systems (ICSs) face growing challenges in delivering population-based high-quality end of life care. The Gold Standards Framework (GSF) helps frontline health and social care staff enhance patient experience and improve care quality. By proactively identifying patients in their last year of life and planning their care, services can improve outcomes, support caregivers, and reduce unnecessary interventions – all while making more effective use of limited resources. Many report cost savings through fewer hospital admissions and crisis care.
How GSF can help your ICS
GSF is the only cross-sector service improvement programme in end of life care, delivering sustainable impact across health and social care. With a proven track record spanning decades, we support organisations at all levels – from care homes and GP practices to hospitals and integrated care systems.
GSF can be the support offer you need
We can tailor our support for either face to face, virtual or distance learning to meet your requirements, by:
- Raising EOLC awareness across your local neighbourhood
- Delivering the GSF End of Life Care training programme to a select group of providers (either hospital wards, care homes, domiciliary care or your GP practices
- Provide training on identification to your primary care teams
- Provide training on Advance Care Planning to your community teams
We are in the early stages of exploring ICS accreditation and will be looking for ICS groups to take part in a pilot scheme.
Embedding GSF leads to real change
- In GP practices, staff report that “being able to work together on something with real impact gave immense job satisfaction in a profession under constant scrutiny.”
- In care homes, teams have seen significant benefits, as “staff are more confident in dealing with death and dying, knowing they helped someone live well and die well. Staff retention has improved tremendously.”
- In domiciliary care, employees feel more valued, with one provider noting, “community nurses now actively engage with our teams, which has strengthened collaboration and boosted staff confidence.”
- Families also see the difference, with one visitor to a 6th-time accredited GSF care home stating, “the world needs to know there are good places like this.”
Achieving GSF Accreditation improves the quality of care, team collaboration, and patient experience and improved outcomes, reinforcing your commitment to gold-standard end of life care.
We're ready to work with your ICS to tailor solutions to your needs and help implement system-wide improvements in end of life care.
