If you work in England and are a GSF Coordinator, Facilitator, End of Life Care Lead, Practice or anyone helping to improve end of life care in your area then please register.
The national GSF team are keen to work with all facilitators and end of life leads and clinical champions to help progress local improvements. We are always happy to arrange a telephone appointment to discuss the particular challenges you are facing. We are also looking to share examples of good practice and innovation leading to improvements in care.
Facilitator training days are held on a regular basis and can feature primary care, care homes or both.
