Calling for interest for the next GSF Acute Hospitals Programme - Spring 2016

The GSF in Acute Hospitals programme helps improve the quality of end of life care (EOLC) in the hospital /ward by proactively identifying patients in the last year of life. The programme helps meet national and local CQUIN  targets regarding readmission rates and length of stay and supports hospitals in attaining a positive CQC report in end of life care.

This programme is being delivered in three ways:

  • Open programme where hospitals across the country come together and share the learning experience 
  • NEW commissioned programme where the GSF team come to you! whole hospital training run on site
  • TAILORED programme for hospital Palliative Care Teams

1. Standard Open programmes - facilitators and ward representatives from a number of hospitals gather together for centrally run GSF training workshops x 6 over 2 years and receive all resources, teaching, coaching and evaluation. Some doing whole hospital projects receive visits in addition.

2. NEW Whole hospital Commissioned programme based at your hospital - ONLY for whole hospital/multiple ward projects, in which the GSF team deliver the programme in your hospital to many wards, making it easier for staff, reduced travel and costs and supporting spread across wards.

3. Tailored Palliative Care team training - A tailored GSF Acute hospital training programme to enable and support the palliative care team to spread GSF across the hospital.
