Plan

Key elements in planning include:

  • discussing and planning personal needs, preferences and wishes of patients with an advance care planning discussion - see ACP section
  • as a clinical care team, planning patient's clinical care and discussion of management options. Use of the GSF 7 Cs helps primary care teams plan ahead. This includes:
    • This is often undertaken at a multi disciplinary team meeting.
    • It also includes planning for out of hours care by sending a handover form to the OOH's provider (SCR 2 or local alternative)
    • anticipatory prescribing especially over a bank holiday weekend (see guidance for just in case boxes).
    • Other planning tools include the use of cross boundary communication such as passport information
    • Use of the Needs Support Matrix
