National Primary Care After Death Analysis Snapshot (Key Findings / Executive Summary)

The Department of Health Quality Markers and Measures for end of life care (QM2.11) 2009 recommends all practices audit their care and create an action plan using an audit tool such as the GSF After Death Analysis (ADA). ADA is available as part of the training programme in Primary Care and Care Homes and can be separately commissioned.

The largest ever national snapshot of end of life care in primary care using the GSF ADA tool is now available, involving 15 PCTs, 600 practices and data from 4,500 patients, assessing every death in February and March 09 in every practice in those PCTs.

Facilitators and end of life care leads who were not part of the project may wish to consider commissioning an ADA audit in future. Many practices who have used ADA are now doing the Quality Improvement Training Programme

The Gold Standards Framework is an excellent template for systematic care for all patients nearing the end of life. As a GP, I found it ensured our patients received the quality of end of life care they deserved, making my life as a GP so much more fulfilling.

DH Quality Markers

The GSF Team welcomes the publication of the Department of Health Quality Markers and Measures for End of Life Care on 29th June 2009. In particular it is exciting to see a new section of Quality Markers for District/Community Nursing Services.

GSF is recommended throughout as well as the GSF audit tool ADA which can be used to benchmark or monitor improvements in care. These markers and measures of quality will be used in the future by commissioners to monitor the care providers are able to give.

The patient keyworker concept is advocated for all organisations in this document. The exact meaning of this concept is not entirely clear and the GSF team would welcome feedback on examples of good practice in terms of how the 'keyworker' role differs from good standard care by a GP or district nurse.
