The role of the practice coordinator is to encourage and oversee the implementation of GSF within the practice. We recommend that the co-ordinator works alongside a practice clinical lead (GP or district nurse) and with the support of the practice team. The co-ordinator should link with local area leads for end of life care. (Check with your community nurse specialist who does this locally).
