What Information Does ADA Collect
ADA collects the following information:
- Demographics including diagnosis, gender, preferred place of care, actual place of death - (No patient identifying data are included)
- Information on communication and coordination of care, including services used - (Relates to QOF, DH markers and GSF Foundation level)
- Care planning, symptom control assessment, continuity of care, out of hours
- Carer support and care in the dying phase - (Relates to DH markers, GSF Higher level)
- Number of crisis admissions, hospital bed days, reasons for not achieving preferred place of care
- Reflective practice section using the ‘traffic lights’ system - (relates to DH markers, GSF advanced level)
