The After Death Analysis (ADA) evaluation audit is now based at the GSF Centre CIC.
**Breaking News**
The National Primary Care Audit is well underway. It considers deaths during February and March 2009 across 15 PCT areas and looks at the care provided for about 4000 patients. Initial funding for this ADA was provided by the Department of Health and additional PCTs have joined. Reporting will be completed by November 2009 with a further evaluation of the impact on improving services in 2010.
Overview of Development
The After Death Analysis (ADA) tool was developed as a means of more effectively evaluating the GSF and relating it more specifically to patient outcomes as well as processes. It also incorporates Significant Event Analysis reflective practice using a 'traffic lights' approach. ADA has been used for primary care and for care homes. Further developments are looking at use in other settings. Originally a paper version, ADA is now a simple to use online tool. Each ADA takes about 10-15 minutes to complete once the information is to hand.

ADA has been extensively researched and evaluated. It has been used extensively in comparative mode exploring the impact of implementing the GSF by asking each practice/home to compare the care of five patients/residents who died before, and five patients/residents who died after implementation of GSF. This highlights areas of improvement in practice and resident outcome. The primary care ADA has been evaluated in a study commissioned by Walsall tPCT (2007) and was piloted for Phase 11 practices from January 2007. The care homes version has been piloted from Phases 2 and 3 of the Care Homes Programme (2007), and the initial online improved version tested by some in Phase 4.
