Building on the success of the primary care and care homes programmes, the National GSF Centre has developed an adapted programme for acute hospitals based on the same principles, toolkit and resources. We've been running a pilot programme involving 16 hospitals (including 3 outpatient units) and the early signs are very encouraging with a report, which will be available here, due end 2010.
Aims of the GSF Acute Hospital Programme:
- To improve the quality of care for all people receiving hospital care in the final year of life
- To improve the cross boundary coordination of care for these patients, before, during and after hospital admissions
- To reduce hospitalisation by reducing length of stay, rapid discharge and admission avoidance thereby reducing costs and improving cost effectiveness

What do you need to do to get involved?
- Senior Manager approval
- Board approval agreed
- Access to training venue and time
- Evaluation feedback
- Agreement to share findings, reflections and attend meetings
- Payment according to local requirement
The Acute Hospitals Programme (Phase 3) will be starting in January 2012.
